Update (April 09, 2019) – Scholarship Submission Deadline Dates
The new form for the 2019 Scholarship/Health Rewards Program is now posted on the scholarship page. The 2019 version of the form must be used and all credits completely filled in with the dates if applicable. Do not leave the credits section blank as we will not be searching our numerous documents to find your credits. We will only be confirming your credits you have listed. Failure to fill in the credits on the form may result in disqualification.
