Update (August 18, 2020) – All Potential Applicants Read
The program will operate as usual in 2020. The deadlines have been moved up one month. The form has been updated and all of the information with the new dates are below. The new form for the 2020 Scholarship/Health Rewards Program is posted below in the first bullet point of the “Program Overview & FAQ’s” shortly. The 2020 version of the scholarship form must be used and all credits completely filled in with the dates if applicable.
Do not leave the credits section blank as we will not be searching our numerous documents to find your credits. We will only be confirming your credits you have listed. Failure to fill in the credits and other information on the form may result in disqualification.
